Changes to global flow configuration definitions now reach running flows.

Global flow configuration changes reach running flows: Creating, editing, enabling or disabling a global flow configuration definition did not change what an already deployed flow saw until the backend's cache expired, which could take up to a week, and re-deploying the flow did not help. The database trigger that notifies the backend of such changes was never installed, because its installation failed silently during server startup. It is now installed once the server has fully started, so definition changes apply to running flows right away, and a failed installation is written to the log as an error.
